Enrollment Instructions for Accepted Transferees A.Y. 2017-2018

INSTRUCTIONS FOR ACCEPTED TRANSFEREES (from other UP Campuses/other Universities)

 

1. Get referral slip from Ms. Nina R. Domingo (starting on Monday, July 10, 2017). Confirm your intention to enroll in BS Tourism by sending an email at upaitois@yahoo.com or call 981-8500 loc. 2799.

2. Present the referral slip to your former college/university and secure:

2.1 Permit to transfer

2.2 College/University clearance

2.3. Honorable Dismissal (transferees from other Universities)

2.4 Official Transcript of Records

*Note: Always photocopy all requirements for submission.

3. Submit to the University Registrar's Office (Admissions Sections) the original copies of #2, plus NSO Birth Certificate and ID picture (2 copies). The Admissions Section will issue the University Admission Slip.

4. Proceed to CRS (2nd floor, University Registrar's Office) and present your University Admission Slip and get PIN for your CRS account. If possible, pre-enlist online and print your Form 5A.

5. On August 2 (Wednesday), submit to AIT the following:

5.1 University Admission slip

5.2 Permit to Transfer (photocopy)

5.3 College Clearance (photocopy)

5.4 Student Directory

6. ENROLLMENT

6.1 Present to adviser your Form 5A. Proceed to enlistment (c/o Ms. Domingo) and enlist in tourism subjects as suggested by the adviser. For GE subjects, go to respective colleges offering the subject and enlist in subjects needed. Having been enlisted in all subjects you need, proceed to checking/validation, assessment and printing of Form 5.

6.2 Post Advising

6.3 Proceed to designated areas for PAYMENT.
